The Entire Healthcare System In The US Is About, To Collapse, Doctors & Nurses Warn

In yet more dire outbreak-induced medical supplies shortage news, the federal government’s own emergency stockpile of respirator masks, gloves, and ventilators is already nearly depleted. Two Homeland Security Department officials told the Washington Post that crucial supplies kept in the Health and Human Services Department’s Strategic National Stockpile are woefully low and will run out amid the pandemic.

“The stockpile was designed to respond to a handful of cities. It was never built or designed to fight a 50-state pandemic,” one official said. “This is not only a U.S. government problem. The national supply chain has already broken down, indicated by what now seems like daily stories of hospital staff in hard-hit major cities having to reuse protective gear, and in other instances actually attempt to make their own out of things like trash bags and household items.

Though in reporting on the federal emergency stockpile crisis The Washington Post and others are emphasizing rampant price gouging as driving it, creating “a Wild-West-style online marketplace for bulk medical supplies dominated by intermediaries and hoarders who are selling N95 respirator masks and other gear at huge markups” — as the Post put it, it remains that the national shortages are rooted in over-reliance on Chinese manufacturing, which itself in the opening months of this year was ravaged by the coronavirus outbreak, causing the shuttering of factories and disruption of ports.

The resultant huge drop in medical supply imports into the US (a drop in up to over half normal numbers in the case of crucial supplies like the N95 mask), led to the emergence of instances such as what the AP earlier alarmingly detailed“Doctors, nurses and first responders in the U.S. are resorting to spraying their masks with bleach at the end of each day and hanging them up at home to dry to use for another day, according to the American College of Emergency Physicians.” 

All of this has also led to a deeper questioning of the White House narrative in terms of its ability to tap necessary emergency supplies

President Trump said during Tuesday’s White House briefing that the administration has nearly 10,000 ventilators on reserve and that authorities are ready to deploy the lifesaving equipment rapidly to coronavirus hotspots in coming weeks. He also said that large amounts of PPE were being shipped directly from manufacturers to hospitals. But the DHS officials said the stockpile has not been able to handle the load.

Hospitals and states face a real risk of running out of supplies, one of the officials said. “If you can’t protect the people taking care of us, it gets ugly.”

Meanwhile, FEMA officials have indicated the federal government has over $16 billion to purchase needed supplies as a fall-back in the anticipation that the Strategic National Stockpile will be exhausted.

FEMA spokesperson Janet Montesi told WaPo: “FEMA planning assumptions for COVID-19 pandemic response acknowledged that the Strategic National Stockpile (SNS) alone could not fulfill all requirements at the State and tribal level.” 

She added: “The federal government will exhaust all means to identify and attain medical and other supplies needed to combat the virus.”

Health professionals are increasingly sounding the alarm over the U.S. healthcare system, warning that the coronavirus outbreak could quickly overwhelm unprepared hospitals without swift action to provide equipment to nurses and doctors. 

“This is a nationwide problem, even on the private side,” an anonymous doctor told NBC News

“No clinic in this country, or hospital for that matter, is going to have enough equipment.”

NBC News reported that around 250 doctors and nurses responded to an informal survey request and painted a bleak picture of a healthcare system already on the verge of collapse with at least a month to go before coronavirus cases peak in the U.S.—citing, in particular, a lack of personal protective equipment (PPE).

According to NBC News:

Nearly all who responded said there were shortages of PPE in the hospitals, outpatient clinics and offices where they worked.

Many reported being forced to ration or reuse supplies, including surgical and N95 masks, for fear of running out. Many also said they were facing shortages of basic sanitary supplies, including hand sanitizer and disinfectant wipes.

One Philadelphia doctor interviewed by NBC News said her husband—also a doctor—is on the frontlines in the city. 

“I am so scared,” she said. “I can’t even begin to tell you.”

Calls for the federal government to step in and order manufacturers to produce equipment increased Friday, with the union National Nurses United issuing a demand for immediate action to protect healthcare workers. 

“We need to act now and act fast,” Bonnie Castillo, National Nurses United executive director, said in a statement.

“Priority number one is to protect the health and safety of our nurses and health care workers so that they can continue to take care of patients and keep our communities as healthy as possible through this pandemic. If our health facilities no longer stay as centers of healing and instead turn into disease vectors, many more people will needlessly suffer from this terrible disease.”

It’s not just PPE. Testing kits are near-impossible to obtain for hospitals and health professionals, leading to difficult decisions on who to treat, when to treat them, and how to track the virus. In Los Angeles County, health officials are instructing doctors and nurses “to test patients only if a positive result could change how they would be treated.”

As the Los Angeles Times reported:

A front-line healthcare provider who was not authorized to speak to the media and requested anonymity said county doctors are interpreting Thursday’s letter and other advice coming from senior L.A. County public health officials to mean they should only test patients who are going to be hospitalized or have something unique about the way they contracted the virus.

They are not planning to test patients who have the symptoms but are otherwise healthy enough to be sent home to self-quarantine — meaning they may never show up in official tallies of people who tested positive.

The coming crisis, as one nurse in Michigan told NBC News, is the result of a societal failure to prioritize public health.

All Hospital Beds In The US Will Be Filled With Patients ‘By About May 8th’ Due To Coronavirus: Analysis

A sobering analysis of how coronavirus is likely to impact the US healthcare system suggests that hospitals will be quickly overwhelmed with patients and that all available beds will be filled by around May 8th if the virus tracks with Italy’s figures and 10% of patients require an ICU.

Of note, the Straits Times reported last week that thousands of people were waiting for hospital beds in South Korea as the disease surges.

Let’s conservatively assume that there are 2,000 current cases in the US today, March 6th. This is about 8x the number of confirmed (lab-diagnosed) cases. We know there is substantial under-Dx due to lack of test kits; I’ll address implications later of under-/over-estimate. 2/n6,3957:03 PM – Mar 6, 2020Twitter Ads info and privacy1,163 people are talking about this

Continued: We can expect that we’ll continue to see a doubling of cases every 6 days (this is a typical doubling time across several epidemiological studies). Here I mean *actual* cases. Confirmed cases may appear to rise faster in the short term due to new test kit rollouts.

We’re looking at about 1M US cases by the end of April, 2M by ~May 5, 4M by ~May 11, and so on. Exponentials are hard to grasp, but this is how they go.

As the healthcare system begins to saturate under this caseload, it will become increasingly hard to detect, track, and contain new transmission chains. In the absence of extreme interventions, this likely won’t slow significantly until hitting >>1% of susceptible population.

What does a caseload of this size mean for the healthcare system? We’ll examine just two factors — hospital beds and masks — among many, many other things that will be impacted.

The US has about 2.8 hospital beds per 1000 people. With a population of 330M, this is ~1M beds. At any given time, 65% of those beds are already occupied. That leaves about 330k beds available nationwide (perhaps a bit fewer this time of year with regular flu season, etc).

Let’s trust Italy’s numbers and assume that about 10% of cases are serious enough to require hospitalization. (Keep in mind that for many patients, hospitalization lasts for *weeks* — in other words, turnover will be *very* slow as beds fill with COVID19 patients).

By this estimate, by about May 8th, all open hospital beds in the US will be filled. (This says nothing, of course, about whether these beds are suitable for isolation of patients with a highly infectious virus.)

If we’re wrong by a factor of two regarding the fraction of severe cases, that only changes the timeline of bed saturation by 6 days in either direction. If 20% of cases require hospitalization, we run out of beds by ~May 2nd.

If only 5% of cases require it, we can make it until ~May 14th. 2.5% gets us to May 20th. This, of course, assumes that there is no uptick in demand for beds from *other* (non-COVID19) causes, which seems like a dubious assumption.

As the healthcare system becomes increasingly burdened, Rx shortages, etc, people w/ chronic conditions that are normally well-managed may find themselves slipping into severe states of medical distress requiring intensive care & hospitalization. But let’s ignore that for now.

Alright, so that’s beds. Now masks. Feds say we have a national stockpile of 12M N95 masks and 30M surgical masks (which are not ideal, but better than nothing).

There are about 18M healthcare workers in the US. Let’s assume only 6M HCW are working on any given day. (This is likely an underestimate as most people work most days of the week, but again, I’m playing conservative at every turn.)

As COVID19 cases saturate virtually every state and county, which seems likely to happen any day now, it will soon be irresponsible for all HCWs to not wear a mask. These HCWs would burn through N95 stockpile in 2 days if each HCW only got ONE mask per day.

One per day would be neither sanitary nor pragmatic, though this is indeed what we saw in Wuhan, with HCWs collapsing on their shift from dehydration because they were trying to avoid changing their PPE suits as they cannot be reused.

How quickly could we ramp up the production of new masks? Not very fast at all. The vast majority are manufactured overseas, almost all in China. Even when manufactured here in the US, the raw materials are predominantly from overseas… again, predominantly from China.

Keep in mind that all countries globally will be going through the exact same crises and shortages simultaneously. We can’t force a trade in our favor.

Now consider how these 2 factors – bed and mask shortages – compound each other’s severity. Full hospitals + few masks + HCWs running around between beds without proper PPE = very bad mix.

HCWs are already getting infected even w/ access to full PPE. In the face of PPE limitations this severe, it’s only a matter of time. HCWs will start dropping from the workforce for weeks at a time, leading to a shortage of HCWs that then further compounds both issues above.

We could go on and on about thousands of factors – # of ventilators, or even simple things like saline drip bags. You see where this is going.

Importantly, I cannot stress this enough: even if I’m wrong – even VERY wrong – about core assumptions like % of severe cases or current case #, it only changes the timeline by days or weeks. This is how exponential growth in an immunologically naïve population works.

Undeserved panic does no one any good. But neither does ill-informed complacency. It’s wrong to assuage the public by saying “only 2% will die.” People aren’t adequately grasping the national and global systemic burden wrought by this swift-moving of a disease.

I’m an engineer. This is what my mind does all day: I run back-of-the-envelope calculations to try to estimate order-of-magnitude impacts. I’ve been on high alarm about this disease since ~Jan 19 after reading clinical indicators in the first papers emerging from Wuhan.

Nothing in the last 6 weeks has dampened my alarm in the slightest. On the contrary, we’re seeing abject refusal of many countries to adequately respond or prepare. Of course, some of these estimates will be wrong, even substantially wrong.

But I have no reason to think they’ll be orders-of-magnitude wrong. Even if your personal risk of death is very, very low, don’t mock decisions like canceling events or closing workplaces as undue “panic”.

These measures are the bare minimum we should be doing to try to shift the peak – to slow the rise in cases so that healthcare systems are less overwhelmed. Each day that we can delay an extra case is a big win for the HC system.

And yes, you really should prepare to buckle down for a bit. All services and supply chains will be impacted. Why risk the stress of being ill-prepared?

Worst case, I’m massively wrong and you now have a huge bag of rice and black beans to burn through over the next few months and enough Robitussin to trip out.

One more thought: you’ve probably seen multiple respected epidemiologists have estimated that 20-70% of the world will be infected within the next year. If you use 6-day doubling rate I mentioned above, we land at ~2-6 billion infected by sometime in July of this year.

Obviously, I think the doubling time will start to slow once a sizeable fraction of the population has been infected, simply because of herd immunity and a smaller susceptible population.

But take the scenarios above (full beds, no PPE, etc, at just 1% of the US population infected) and stretch them out over just a couple extra months.

That timeline roughly fits with consensus end-game numbers from these highly esteemed epidemiologists. Again, we’re talking about discrepancies of mere days or weeks one direction or another, but not disagreements in the overall magnitude of the challenge.

This is not some hypothetical, fear-mongering, worst-case scenario. This is reality, as far as anyone can tell with the currently available data.

Despite repeated attempts to assure the public that the coronavirus pandemic is under control in the United States, the federal government continues to take drastic measures, just in case.

A source close to NBC News says the Federal Emergency Management Agency (FEMA) is getting ready for President Donald Trump to declare an “infectious disease emergency declaration.”

Once this national emergency is declared by the President, FEMA would be able to activate the $34 billion in funds the agency currently maintains. “It’s money that’s sitting there and ready,” said a former FEMA official.

“To me, this is another indication that the president and the White House are finally aware of the gravity of the situation,” said Michael Coen, former FEMA chief of staff. “They need to consider all the tools available to them and have contingencies for action.”

An emergency declaration would unlock the military and billions of dollars in medical assistance

These FEMA funds would assist the administration’s strong coronavirus response, enlisting the military to help out. The emergency declaration would allow FEMA officials to provide disaster relief funding to state and local governments. The CDC has already identified coronavirus cases in fourteen states. This emergency declaration would allow FEMA to target the areas already affected by the virus, allowing for medical assistance teams, mobile hospitals, and military transport to help out with potentially overcrowded hospitals and panicked people.

In order to mobilize the military and activate FEMA’s disaster assistance funds, the Trump Administration would have to use the 1988 Stafford Act. The Stafford Act was used by President Bill Clinton in 2000 to combat the West Nile virus in New York and New Jersey. Up to $5 million in federal aid was used to help these states control mosquitoes and help people recover from the mosquito-borne illness. This time, the declaration could impact multiple states and potentially activate the National Guard across the country.

Health and Human Services (HHS) have already declared this coronavirus strain to be a “public health emergency.” FEMA is currently working with HHS to assist the federal government.

All this is on top of CDC director Dr. Robert Redfield recently telling CNN that, “This virus is probably with us beyond this season, beyond this year, and I think eventually the virus will find a foothold and we will get community-based transmission.”
